Patients will be charged for the services not covered by Alberta Health Services. 

 

• AISH Application- $150 

• Alberta Works application- $70 

• Drivers’ Medical- $100 

• No Health card or inactive- $50 

• Medical certificate/sick note- $25 

• Disability application- $150 

• No Show counseling appointment-$50

• No-Show Addiction physician appointment- $100

• No Show Integrative Medicine and Women's Health- $250

• Initial Integrative and Functional Medicine Assessment- Fee Waived

• Followup Integrative and Functional Medicine assessment - $500 (Includes case preparation)

• IV Therapies - $120 base rate + cost of nutrients (Iron on prescription)
